Your WVPP can look complete and still get cited. That is how Cal/OSHA runs the inspection.
You hand over the plan. The inspector reads it, then walks the floor and asks your employees two questions. Were you asked to help identify hazards. Do you know how to report. If the team says no, you fail Labor Code §6401.9(c)(2)(B). Paper does not save you when people cannot answer.
What Section 6401.9(c)(2)(B) actually says
The law is direct.
Effective procedures to obtain the active involvement of employees and authorized employee representatives in developing and implementing the plan, including, but not limited to, through their participation in identifying, evaluating, and correcting workplace violence hazards, in designing and implementing training, and in reporting and investigating workplace violence incidents.
Three tests in one sentence. Active involvement in development, in implementation, and across four areas: hazards, corrections, training, and reporting.
Most PDFs answer none. A signature page is not a procedure. Inspectors ask for dates, roles, how input was collected, and what changed. A manager signature has no answer.
How inspections fail in three minutes
The order is predictable. Plan first, then log, then corrections, then training, then interviews. Interviews are where the PDF dies.
The answers sound the same.
Were you asked to help find hazards. No, not really. There was a meeting but we were not asked.
Do you know how to report. I would tell my manager, I think. There is a number but I have not used it.
Have you seen the plan. I heard we have one. It is in the office.
None of this requires hostility. Calm answers still produce a citation if the procedure was missing. You cannot backdate involvement.
Why 95 percent of templates fail
Almost every template we review shares the same gaps. Check your file against these.
Site generic. SB 553 requires an assessment per worksite under Section 6401.9(c)(2)(F) and correction with verification under (G). A plan with no entrances, cash points, isolated shifts, or Type 1 to Type 4 profile for this address is a placeholder. A Fresno yard is not a Bakersfield clinic. Copy and paste misses that.
Input as a line item. A signature page is not a procedure to obtain active involvement. No survey, no response log, no injection of feedback into the text.
No correction loop. Hazard tasks need an owner, a due date, and a verification date. A section that says management will correct hazards promptly without a ticket and a sign off does not satisfy (G). Inspectors ask to see hazard, correction, and who verified it.
Mixed exports. Plan, log, and training roster in one ZIP leaks names or leaves the log incomplete. That violates the five year duty under Section 6401.9(d). Subilu exports three separate files. PDF or DOCX for the plan, CSV or PDF for the log with private and employee safe views, CSV for training.

The fix is not better prose. It is a record that employees participated and the plan changed because of them.
Push to phone. After you answer the wizard, the system sends a short survey to staff phones. What hazards do you see. How would you report a threat. Two minutes, plain language, timestamped and tied to worksite.
Phone survey. Staff answer privately, no manager filter. You get a response log with role and time that proves you asked.
Inject into the plan. Flags from responses are injected into the WVPP and assessment. Lighting flagged by the closing crew becomes a line in the assessment. That injection is dated.
Hazard tasks. Each hazard becomes a task with owner, due date, and verification. Mark it corrected, a second person verifies, Appendix B shows hazard, action, and date. That satisfies (G).
Three exports. Push record, survey results, and correction log live in the appendices. You export the WVPP as PDF or DOCX, the log as its own CSV or PDF with formula guard and BOM for Excel, and the training roster as its own CSV with 11 columns. Separate files, separate retention. That is the five live features: WVPP 21 plus 4, silent log dual view, hazard tasks, mobile training, and export suite.
